Meet Elisa
Founder, Hometown Health and Wellness
Elisa is a Board-Certified Family Nurse Practitioner and Purdue University Master’s graduate who has proudly served the La Porte community for more than 20 years.
Throughout her career, Elisa has remained committed to delivering high-level clinical care rooted in genuine relationships. As a Direct Primary Care provider, she practices with intention — prioritizing time, accessibility, and continuity so patients feel heard, respected, and fully supported.
Her approach blends advanced primary care with whole-person wellness, helping individuals and families navigate both immediate health concerns and long-term wellbeing. Elisa believes healthcare should be clear, compassionate, and centered around the patient — not insurance limitations or rushed appointments.
Rooted deeply in this community, she is passionate about providing accessible, affordable care close to home. Through Hometown Health and Wellness, Elisa has created a space where medical excellence meets true hometown connection.
Outside the clinic, Elisa enjoys life with her husband, three adult children, and two grandchildren. She spends her time reading, staying active at the gym, enjoying the outdoors, and making lasting memories with her family.

Meet Abbi Kroll
Family Nurse Practitioner
Abbi is a Board-Certified Family Nurse Practitioner who is proud to serve the La Porte community she has called home her entire life.
With five years of experience as a Family Nurse Practitioner — primarily in the Emergency Department — and a total of ten years in emergency and hospital-based medicine, Abbi brings a broad and well-rounded clinical background to Direct Primary Care. Her experience in high-acuity settings has shaped her ability to think critically, respond confidently, and care for patients across all stages of life.
At Hometown Health and Wellness, Abbi is passionate about transitioning from fast-paced hospital care to relationship-based medicine — where she can spend meaningful time with patients, focus on prevention, and support long-term health. She is committed to building trusted partnerships and helping individuals and families improve their overall quality of life.
Deeply involved in her community, Abbi has served as a volunteer firefighter for five years and in her church’s children’s ministry for nine years. Outside of the clinic, she enjoys life with her husband, Marcus, and their two children. She loves staying active outdoors — especially running, biking, and swimming.

Christian Inspired Yoga is a Vinyasa-style class that blends breath and movement in a thoughtful, flowing sequence. Guided by a Christian-themed playlist, the practice offers both physical challenge, and spiritual connection. You'll be encouraged to move with intention, finding moments of reflection and gratitude throughout the class.
Open to all levels, this class provides options and modifications so beginners feel supported and experienced practitioners can deepen their practice.
During final relaxation (savasana), Janet offers a silent prayer over each participant while placing a cool, lavender-infused washcloth gently on your forehead - creating a peaceful, grounding experience to close your practice.
Though rooted in Christian beliefs, this class welcomes individuals of all backgrounds and spiritual paths who are open to a mindful, heart-centered experience.
This class does require 24 hour notice to cancel. If canceled less than 24 hours prior to class, refunds will not be given.

This hatha yoga class invites you to explore the connection between energy, movement, and breath in a way that's accessible for all levels, especially beginners. Each practice is guided by the energy of the group and the season ~ some days more gentle and grounding, others with a little more movement and flow.
You'll be encouraged to listen to your body move at your own pace, and make each pose your own. We'll explore mindful alignment, breath awareness, and moments of stillness ~ creating balance between strength and ease.
No experience is needed ~ just an open mind and willingness to show up as you are. You'll leave feeling grounded, renewed, and more connected to yourself ~ body, mind, and spirit.
